(394) Evaluating Heart Transplant Outcomes Utilizing the Sherpapak Heart Storage System

چکیده
PurposeMitigating cold ischemic myocardial injury during organ transport may increase cardiac function and improve patient outcomes post-transplant. SherpaPak (SP) is a storage transportation device that maintains the donor heart at constant, hypothermic temperature lower risk of when compared to traditional on ice.MethodsWe retrospectively analyzed adult patients who underwent isolated transplantation University Kansas Medical Center from June 2020 2022. Baseline characteristics were between utilized SP ice. Outcomes groups using Wilcoxon tests for continuous, non-normal variables Fisher's exact or chi-square tests, as appropriate, categorical variables. Multivariable linear regression was used analyze association length stay with clinical predictors.ResultsA total 85 included in study, 25 (29.4%) hearts transported 60 (70.6%) No significant differences noted baseline groups. The group trended towards having higher average age (32 yrs vs. 28 yrs; p = 0.052), time (197 min 177 min; 0.193), bypass (136 127 0.194). Average vasopressin dose (56 units 146 units; 0.034). On multivariable analysis, use associated reduced (11 days vs 19 days; 0.006) after controlling age, time, dose, cause cardiomyopathy, UNOS status transplant, gender matching. difference 30 transplant rejection, readmission mortality groups.ConclusionDespite older longer times, usage shorter post-transplant standard storage. Insurance Coverage and Heart Transplant Outcomes.
Heart transplantation in the United States has largely been restricted to patients with adequate health insurance coverage to optimize recipient outcomes, particularly in the context of the scarcity of donor organs. Public health insurance programs, such as Medicaid and Medicare, have been instrumental in providing access to heart transplantation and other solid organ transplants for patients u...
متن کاملA Physical Heart Failure Simulation System Utilizing the Total Artificial Heart and Modified Donovan Mock Circulation.
With the growth and diversity of mechanical circulatory support (MCS) systems entering clinical use, a need exists for a robust mock circulation system capable of reliably emulating and reproducing physiologic as well as pathophysiologic states for use in MCS training and inter-device comparison. We report on the development of such a platform utilizing the SynCardia Total Artificial Heart and ...
متن کاملThirty-day outcomes in Medicare patients with heart failure at heart transplant centers.
BACKGROUND Heart transplant centers are generally considered "centers of excellence" for heart failure care. However, their overall performance has not previously been evaluated in a broad population of elderly patients with heart failure, many of whom are not transplant candidates. METHODS AND RESULTS We identified >1 million elderly Medicare beneficiaries who were hospitalized for heart fai...
متن کاملCongestive heart failure in renal transplant recipients: risk factors, outcomes, and relationship with ischemic heart disease.
Cardiovascular disease (CVD) is the major cause of death in renal transplant recipients (RTR). Several cohort studies have examined CVD in RTR, but none have addressed the development of congestive heart failure (CHF). CHF would hypothetically be a frequent and prognostically important event in RTR. A retrolective cohort study was, therefore, conducted in two Canadian centers to describe the in...
متن کامل445Epidemiology and Outcomes of Clostridium difficile Infections in Heart and Heart-lung Transplant Recipients
Background. Clostridium difficileinfection (CDI) is an important cause of diarrhea in transplant recipients, and often complicated by the use of intense immunosuppression and need for antibiotic prophylaxis. We aimed to investigate the epidemiology of CDI in heart and heart-lung transplant (HT) recipients and assessed its impact on outcomes. Methods. This is a retrospective study of HT recipien...
